Depression self-assessment tool

This self-assessment isn’t designed to diagnose depression — only a physician or mental health professional can do that. But if you think you may be depressed, take the assessment and share the results with your doctor. Keep in mind that the tool is intended for adults 18 years and older.

Over the past 2 weeks, how often have you been bothered by any of the following problems?
